In order to make your scheduled appointment run as efficiently as possible, we have provided our patient forms in a downloadable format so you can complete them ahead of time and bring them with you on the day of your appointment. There are different patient forms available for both new and returning clients as well as different forms to fill out if you are receiving a certain procedure such as an injection or an electromyogram (EMG). The patient forms generally include patient contact information as well as basic information regarding medical history and an overview of the pain symptoms experienced. There is also an outline of company policies that mainly addresses the issues of cancellations and rescheduling appointments as well as an outline of patient rights and responsibilities. Patients may also be required to fill out a medication agreement to help both the patient and physician to comply with the law regarding pain-control medications. The patient forms may also include an authorization for the disclosure of certain health information which is often necessary if a patient is working with other doctors to aid in their pain management such as physical therapists.

Depending on your appointment type we will request that you arrive between 5 and 30 minutes before your scheduled appointment time. We ask that patients print out, complete, and bring the appropriate patient forms with you to your appointment. You may be seen quicker if your patient forms are completed prior to your arrival. Unlike many medical practices we do not over-book patients. Therefore, it is very important that, barring any unforeseen emergency, you call us at least 24 hours prior to your scheduled office appointment if you need to cancel or reschedule. This way we will have time to put another patient that is in need of care in that time slot. It is our goal to meet the needs of our patients, and we will thus make every effort to schedule your appointments as efficiently as possible. In return, it is your responsibility to make every effort to keep your scheduled appointments and to arrive promptly at the time instructed.
We provide the online patient forms for your own personal convenience so that you may complete them in the timeframe that is best for you.
